1) Goto Government of India website (https://passport.gov.in/oci/welcome)
- In Miscellaneous services, select OCI RE-ISSUE NEW PASSPORT
- Fill both forms A && B
- Print the Application and Affix the photo
- Upload Photo and Signature in Government website
- Get the web reference Number
2) Goto CKGS Website
- Goto OCI RE_ISSUE NEW PASSPORT
- Download Parent Authorization Form (https://www.indianembassyusa.gov.in/)
- Select mode of delivery (Shipping/Walk-in)
- Pay fees ($17 CKGS fees + 3$ Welfare fund)
- Send Physical copy to CKGS
